MJHS offers individualized care that is specially designed to meet each client’s specific needs. The services offered at the MJHS Adult Day Health Care centers include:

The services offered at our Adult Day Health Center are designed for adults. We accept Medicaid, all managed care Medicaid insurances, private pay and Brooklyn Veterans Administration Beneficiaries. MJHS arranges transportation to our center and back to your home.

Diabetic Management Program
The Diabetes Management Program has been in operation for many years. It is a voluntary, culturally sensitive, individually tailored program for MJHS Adult Day Health Care clients who have a diagnosis of Diabetes.

The program consists of peer support, health education, nutritional instruction, nursing assessment and activities. The goal of the Diabetic Management Program is to empower the clients with knowledge and the self management skills needed to make better choices so they can safely and better manage their diabetes. There is a weekly mailing of all the covered material to the significant other of the cognitively impaired clients.

The program is very successful. 90% of our diabetic clients have HgA1C levels within acceptable range and our annual hospitalizations due to diabetes related illness is always in the single digits.

Pre-Diabetic Management Program
The Pre-Diabetes Management Program has been successfully in operation for 2 years. The goal of the Pre-Diabetes Management Program is to identify our clients that are at risk for developing diabetes or are in a pre-diabetic status, provide the “at risk clients” the support of their peers and provide them with the education and tools they need to safely manage their pre-diabetic status and prevent further disease progression.

The clients receive weekly professional instruction from an RN, nutritionist and a certified diabetic educator. The education includes monitoring of blood glucose, medication management, food preparation demonstrations, trips to supermarkets.

Participation is very high. The program is very successful as there has been no conversion from Pre-Diabetes to a diagnosis of Diabetes

Multiple Sclerosis Support Group
The MS group has been together for a very long time, several clients have been attending for more than 10 years. The group is led by a trained, licensed social worker. The MS support group provides our clients with MS an opportunity to share their MS experiences with those who can understand and identify with them.

Our MS Support Group meets weekly. The meetings are designed to be social, educational, and lots of fun. The MS group has evolved into a cohesive unit; there is an atmosphere of camaraderie among this special group of clients. Our MS clients truthfully rejoice in each others successes and empathize with each others set backs.

Annual Influenza Vaccination Campaign
The annual Influenza Vaccination Initiative has had extremely high client participation. We have historically immunized 20% more of our clients than the national average. Flu vaccines are distributed annually in a health fair fun atmosphere.

Our RNs distribute the vaccinations to both clients and staff together. The vaccination is followed by some TLC (cookies and hot tea) this gives the staff the opportunity to safely observe the clients while they relax and enjoy a snack. Every participant receives a goody bag filled with educational material, another treat and some useful items to take home.

Food Preparations/Demonstrations
This monthly group is led by our registered dieticians. They work together with our nursing staff to bring nutrition education to our clients. The clients are provided with education on how to improve their diet, where and how to shop, the nutritional value of the ingredients and methods of preparation. This group is interactive and clients get to sample the delicious food prepared.

Outings and Trips
Clients are taken on frequent trips to restaurants, parks, concerts, museums, shows and to a variety of stores. These outings keep our clients connected to the world around them and connected with the things they love. The outings are staffed with a team of professionals and client safety is paramount.

Multi-Cultural Celebrations
MJHS’s commitment to multiculturalism is reflected in the variety of events we host celebrating the center’s cultural, racial and religious diversity. There is something for everyone.
