Heart Failure Disease Management

To reduce the risk of re-hospitalization for heart failure patients within 30 days of their discharge, MJHS has developed an innovative and highly successful heart failure disease management program through First to Care Home Care, Inc., our joint venture with Maimonides Medical Center.

Patients in our program have experienced dramatically less re-hospitalization than the national average. Our program is so successful that we are the first home health agency in the nation to achieve Joint Commission Certification for heart failure disease management.

This breakthrough program is just one example of MJHS’ passionate commitment to providing the absolute best in medical care and patient outcomes.
