The Upper Manhattan Partnership for Senior Independence was created as a grassroots collaboration among leading service providers dedicated to caring for the aging population in our community.
How Can We Help?
Our primary focus is to provide the support needed to help you, a loved one, or a friend enjoy a healthier and more satisfying life in the comfort of your home by:
- Offering Case Management
- Coordinating meal delivery through Meals on Wheels
- Providing an in-home assessment to see if you qualify for EISEP home care (Expanded In-Home Services for the Elderly Program)
- Utilizing HomeMeds, a medication safety program
- Connecting you with social daycare programs in your area
- Making referrals to community resources and providing information
