The Hospice Enrollment Process

The process begins with a referral, which can be made by your treating physician, a hospital, or even a family member or friend. Once the referral is made, the hospice team coordinates the next steps, ensuring the process is easy and tailored to your specific needs.

Next, a hospice nurse will visit you to confirm eligibility and identify your care requirements. During this visit, they will discuss your medical condition, goals of care, and current support system. They will also gather details about your physician(s) and address any questions or concerns you or your loved ones may have about hospice care. This initial assessment is crucial for understanding your unique needs and preferences, forming the foundation of your personalized care plan.

As part of the process, the hospice team will contact your treating physician to confirm your prognosis and ensure they are aware of your decision to stop seeking curative treatment. This collaboration ensures everyone involved in your care is on the same page and working together to provide the best possible support.

Once your eligibility is confirmed, you will be enrolled in hospice care. 

After Enrolling in Hospice Care

Your hospice team will develop a personalized care plan. This plan is created in partnership with you, your family, and, if you wish, your physician. It addresses your physical, emotional, spiritual, and social needs, ensuring comprehensive and compassionate support is provided.

Your plan of care will include various types of support provided by our interdisciplinary team of experts:

  • Hospice Physician: Provides medical guidance for your interdisciplinary care team and participates in care planning. They may make home visits as needed and, upon request, can assume the role of primary care physician.
  • Hospice Nurses: Regularly monitor your condition and provide medical care, ensuring your physical needs are met.
  • Social Workers: Provide emotional support, counseling, and resources to help you and your loved ones navigate this journey.
  • Chaplains: If desired, offer spiritual guidance tailored to your beliefs and values.
  • Home Health Aides: Assist with personal care tasks such as bathing and grooming, helping you maintain dignity and comfort. The number of hours provided varies depending on the patient’s condition.   
  • Volunteers: Offer companionship and help with light tasks, 

The frequency and length of these visits will evolve as your needs change, ensuring you are supported throughout your end-of-life journey.
