  • Registered Nurse — your primary clinical contact, visiting regularly to monitor your condition, manage medications, and adjust your care as your needs evolve
  • Social Worker — helps you and your family navigate the practical and emotional challenges that come with serious illness, and connects you to community resources and support
  • Spiritual Care Advisor — provides compassionate, non-denominational spiritual and emotional support for patients and families of all faiths and backgrounds
  • Creative Arts Therapist — uses music, art, and other creative modalities to support emotional expression, reduce anxiety, and enhance quality of life
  • Home Health Aide — assists with personal care and daily needs in the comfort of your home
  • Volunteers — offer companionship, practical support, and relief for family caregivers
