With a focus on providing personalized support and enhancing the quality of life, we offer you or your loved one two great options for receiving long-term care. You can access the care you need in the comfort of your home, with support from one of our long-term care plans, or receive expert, compassionate support in one of our two Centers for Rehabilitation and Nursing Care.

We can help with the following:

  • Caregiver education and resources.
  • Accessing expert medical care tailored to your individual needs 

If you’re looking for inpatient long-term care for a loved one, MJHS Menorah Center in Brooklyn and MJHS Isabella Center in Upper Manhattan are excellent options. If you or a loved one prefer to receive care at home, Elderplan/HomeFirst—a participating agency of MJHS Health System—offers two comprehensive long-term care plans that provide essential services and support for those who qualify.

Oceanfront Location

Situated on 10 scenic oceanfront acres in Manhattan Beach, Brooklyn, our not-for-profit skilled center provides expert long-term care services in a peaceful, supportive setting.

Specialized Programs

Our award-winning memory care program supports residents with Alzheimer’s and other forms of dementia, and cognitive impairment. The MJHS Memory Care Residence also offers specialized support for those needing advanced care. Residents with a prognosis of six months or less also have access to on-site end-of-life care, including the 16-suite MJHS Mollie and Jack Zicklin Hospice wing.

MJHS Isabella Center

Located on 4 beautifully landscaped acres in the thriving Washington Heights section of Upper Manhattan, MJHS Isabella Center provides expert long-term care services in an engaging and supportive environment.

Ventilator Support

We are one of the few skilled centers in New York City that has certified ventilator-dependent beds. As part of our exceptional respiratory program, we also have a dedicated step-down unit for patients being weaned off ventilator support.

Independent Living

Located on the campus of MJHS Isabella Center, Isabella House is an independent living, high-rise apartment building for adults 62+. Each remodeled studio or one-bedroom apartment is moderately priced, bright, spacious, and offers stunning skyline views. Rent includes numerous amenities, including two meals per day and free parking.
