Your care team includes:

  • Registered Nurses (RNs): Our Registered Nurses are at the heart of your care team. They oversee your overall health, provide hands-on care, coordinate with your doctors and other team members, and closely monitor your progress. Focusing on clinical expertise and compassionate support, they ensure your care plan addresses your individual needs.
  • Social Workers: Our Social Workers are there for our patients and their families to help address the emotional impact caused by a sudden change in health status or new limitations that might have resulted from a prolonged illness, injury, chronic disorder or neurologic condition. Disease-Specific Expertise

If you have Congestive Heart Failure (CHF), Chronic Obstructive Pulmonary Disease (COPD) or Diabetes, our skilled nursing team has expertise in providing disease-specific care for these conditions. We provide education and support, helping you better understand and manage your condition. We also teach your family how to assist in your care, giving them the tools and confidence to support your chronic condition.

Exceptional Care Coordination

Our skilled nursing team works closely with our short-term rehabilitation therapists, including physical (PT), occupational (OT), and speech therapists (ST), as well as our social workers. We collaborate with you and your treating physician to set goals and create a fully integrated care plan.
