We focus on understanding who you are beyond your illness, taking a holistic approach to developing the most effective plan of care.

Depending on your unique situation and preferences your plan of care may include the following:

  • Visits from members of the hospice Interdisciplinary team of experts, including a Hospice Physician, Registered Nurse, Social Worker, Spiritual Care Advisor, Creative Arts Therapist, Home Health Aide, and Volunteers.
  • Managing the physical symptoms of your illness.
  • Providing emotional and spiritual support for you and your loved ones.
  • Supplying medications and durable medical equipment (DME), including oxygen and other needed supplies.
  • Physical therapy, occupational therapy, respiratory therapy, speech and language therapy, dietary counseling, and laboratory services are also provided as needed.
