This list represents some of the common qualifying indicators for admission to our Certified Home Health Agency.

    • Anemia, B12 injections
    • Care of indwelling urinary catheter
    • COPD
    • Diabetes
    • Drainage tubes
    • Exacerbation of chronic illness
    • Heart disease
    • High blood pressure
    • Hip or knee surgery
    • Hospital re-admissions
    • Injections and infusions
    • Kidney disease, dialysis
    • Lower extremity fractures
    • Multiple emergency room visits
    • New medication and/or medication management
    • Newly diagnosed chronic illness (Parkinson’s)
    • Ostomy care
    • Pain management
    • Post-operative care
    • Rehabilitation therapy
    • Stroke (new or symptoms of late effects)
    • Trach care, oxygen
    • Unsteadiness, frequent falls
    • Wound care
